Na osnovu člana 13. Zakona o principima lokalne samouprave u Federaciji Bosne i Hercegovine (‘“Službene novine FBiH“, broj 49/06 i 51/09), člana 8. Zakona o ministarskim, vladinim i drugim imenovanjima F BiH („Službene novine F BiH“, broj: 12/03, 34/03 i 65/13), člana 64. stav (8) Zakona o zdravstvenoj zaštiti („Službene novine FBiH“, broj 46/10 i 75/13), člana 6. stav 1. tačka 2. i 19. Statutarne odluke o organizaciji Grada Gradačac u skladu sa Zakonom o Gradu Gradačac (“Službeni glasnik Grada Gradačac“, broj 1/19), člana 10. Odluke o utvrđivanju standarda i kriterija za imenovanja/nominiranja na upražnjene pozicije u reguliranim organima Općine Gradačac („Službeni glasnik Općine Gradačac“, broj 9/17) i člana 1. Odluke o objavljivanju javnog oglasa za izbor i imenovanje predsjednika Upravnog odbora Javne ustanove „Apoteka Gradačac“ („Službeni glasnik Grada Gradačac“, broj 5/22), Komisija za izbore i imenovanja Gradskog vijeća Grada Gradačac, o b j a v l j u j e JAVNI OGLAS za izbor i imenovanje predsjednika Upravnog odbora Javne ustanove „Apoteka Gradačac“         

Objavljuje se javni oglas za izbor i imenovanje predsjednika Upravnog odbora Javne ustanove „Apoteka Gradačac“. 

1. Opis upražnjene pozicije:          Predsjednik upravnog odbora odbora obavljat će poslove u skladu sa Uredbom sa zakonskom snagom o ustanovama (“Službeni list RBiH”, broj 6/92), Zakonom o zdravstvenoj zaštiti FBiH (“Službene novine FBiH”, broj 46/10 i 75/13) i Statutom javne ustanove “Apoteka Gradačac”.

 2. Opći uslovi:Opći uslovi za imenovanje/nominiranje na upražnjene pozicije u reguliranim organima Općine su:a) da je državljanin Bosne i Hercegovine,b) da je stariji od 18 godina,c) da nije otpušten iz državne službe kao rezultat disciplinske mjere (bilo na nivou države ili entiteta) u periodu od tri godine prije dana objavljivanja upražnjene pozicije,d) da se na njih ne odnosi član IX.1. Ustava Bosne i Hercegovine,e) da nije na funkciji u političkoj stranci u smislu člana 5. Zakona o ministarskim, vladinim i drugim imenovanjima Federacije Bosne i Hercegovine („Službene novine Federacije Bosne i Hercegovine“, broj:12/03, 34/03, 65/13),f) da nije stariji od 65 godina na dan imenovanja,g) da se ne bavi samostalno ili u zajednici sa drugim osobama djelatnošću konkurentnom reguliranom organu,h) da nema privatni i/ili finansijski interes u instituciji u koju se kandiduje,i) da nije osuđivan za krivično djelo nespojivo sa imenovanjem na dužnost u instituciju u koju se kandiduje, pet godina od dana pravosnažnosti presude, isključujući vrijeme zatvorske kazne,j) da mu nije izrečena zaštitna mjera zabrane obavljanja aktivnosti, tri godine od dana pravosnažnosti odluke o izrečenoj zaštitnoj mjeri.

 3. Posebni uslovi:(1) Posebni uslovi za imenovanje/nominiranje člana upravnog odbora u javnim zdravstvenim ustanovama i ostalim ustanovama osnovanim od strane Općine Gradačac:a) stepen obrazovanja VSS ili VŠS,b) najmanje 1 (jedna) godina radnog iskustva u struci,c) da nije član nadzornog ili upravnog odbora, odnosno odbora za reviziju privrednog društva,d) da posjeduje komunikacijske i organizacijske sposobnosti,e) da posjeduje sposobnost tumačenja ekonomskih i pravnih analiza i propisa.(2) Prilikom procjene posebnih uslova, kriteriji navedeni pod tačkama d) i e) iz stava (1) ovog člana izrazit će se u bodovima od 1-5. 
4. Dužina trajanja mandata:          Imenovanje predsjednika Upravnog odbora Javne ustanove “Apoteka Gradačac” vrši se do isteka mandata Upravnom odboru koji je imenovan 28.09.2021. godine na mandat od četiri godine. 

5. Naknada za rad:Mjesečna naknada za rad propisana je Odlukom Općinskog vijeća. 
6. Mogućnost ponovnog imenovanja:Nema ograničenja za ponovno imenovanje. 

7. Uslovi službe:Uslovi službe su definisani Odlukom o utvrđivanju standarda i kriterija za imenovanja/nominiranja na upražnjene pozicije u reguliranim organima Općine Gradačac.

 8. Potrebni dokumenti uz prijavu:Uz prijavni obrazac, kandidati su dužni dostaviti:- kraću biografiju sa adresom i kontakt telefonom,- Izvod iz matične knjige rođenih (original ili ovjerena kopija),- Uvjerenje o državljanstvu ne starije od 6 mjeseci od dana izdavanja (original ili ovjerena kopija),- PBA-3 obrazac (prijava mjesta prebivališta), (original ili ovjerena kopija),- Ovjerenu kopiju diplome o stečenoj stručnoj spremi,- Uvjerenje o radnom iskustvu u struci (original ili ovjerena kopija),- Ovjerenu izjavu da nije otpušteni iz državne službe kao rezultat disciplinske mjere na bilo kojem nivou vlasti u Bosni i Hercegovini u periodu od tri godine prije dana objavljivanja ovog oglasa,- Ovjerenu izjavu kandidata da se na njega ne odnosi član IX. tačka 1. Ustava Bosne i Hercegovine,- Ovjerenu izjavu da nije na funkciji u političkoj stranci u smislu člana 5. Zakona o ministarskim, vladinim i drugim imenovanjima F BiH („Službene novine F BiH“, broj: 12/03, 34/03 i 65/13),- Ovjerenu izjavu da se ne bavi samostalno ili u zajednici sa drugim osobama djelatnošću konkurentnom reguliranom organu,- Ovjerenu izjavu da nema privatni i/ili finansijski interes u instituciji u koju se kandiduje,- Ovjerenu izjavu da nije osuđivan za krivično djelo nespojivo sa imenovanjem na dužnost u instituciju u koju se kandiduje, pet godina od dana pravosnažnosti presude, isključujući vrijeme zatvorske kazne,- Ovjerenu izjavu kandidata da mu nije izrečena zaštitna mjera zabrane obavljanja aktivnosti, tri godine od dana pravosnažnosti odluke o izrečenoj zaštitnoj mjeri, i- Ovjerenu izjavu da nije član nadzornog ili upravnog odbora, odnosno odbora za reviziju privrednog društva. 

Prijavni obrazac kandidati mogu pronaći na web stranici grada Gradačac. Tražene izjave kandidati mogu dostaviti kumulativno navedene sa jednom ovjerom koja vrijedi za sve izjave ili svaka izjava pojedinačno ovjerena. U roku od osam dana od dana imenovanja kandidat koji bude konačno imenovan za predsjednika Upravnog odbora biti će dužan dostaviti uvjerenje da nije osuđivan za krivično djelo koje predstavlja smetnju za imenovanje na javnu poziciju, odnosno da mu odlukom suda nije zabranjeno obavljanje aktivnosti vezano za poziciju za koju se kandiduje, u roku od 5 godina od dana izdržane, izvršene, zastarjele ili oproštene kazne. Kandidati zaposleni u organima državne službe koji uđu u uži izbor i budu pozvani na intervju dužni su priložiti rješenje kojim je kandidatu data pismena saglasnost od strane rukovodioca organa državne službe za obavljanje dopunske djelatnosti.Dokaz da kandidat nije osuđivan za krivično djelo od nadležnog Ministarstva unutrašnjih poslova službeno će se tražiti za kandidata koji bude predložen za konačno imenovanje.

 Oglas će se objaviti u Službenim novinama FBiH, jednom dnevnom listu većeg tiraža koji izlazi na području Bosne i Hercegovine, web stranici Grada Gradačac i Radiju Gradačac.

Sa kandidatima koji budu ispunjavali sve potrebne uslove javnog oglasa biti će obavljen intervju.Termin intervjua sa listom kandidata koji ispunjavaju uslove javnog oglasa će biti objavljen na zvaničnoj web stranici Grada Gradačac.

 Oglas ostaje otvoren 15 /petnaest/ dana od dana posljednje objave u javnom glasilu. Prijave sa potrebnom dokumentacijom treba dostaviti lično ili preporučeno poštom na adresu: Komisija za izbor i imenovanja Gradskog vijeća Grada Gradačac, ul. H.K. Gradaščevića br. 74, 76250 Gradačac, sa naznakom „Prijava na Javni oglas za izbor i imenovanje presjednika Upravnog odbora Javne ustanove “Apoteka Gradačac”– ne otvaraj“. Nepotpune, neblagovremene i neuredne prijave neće se uzeti u razmatranje.

Zainteresirana stranka može podnijeti prigovor na konačno imenovanje, ako raspolaže dokazima da u postupku imenovanja nisu ispoštovani principi utvrđeni Zakonom.

Prigovor na konačno imenovanje se podnosi Komisiji za Statut i opća akta Gradskog vijeća, a kopija se dostavlja Ombudsmenu za ljudska prava BiH.
